What Are Transition Services?
Transition services are a coordinated set of activities designed to facilitate a student's movement from school to post-school activities. Under IDEA, transition planning must begin no later than the IEP in effect when the student turns 16 (some districts begin at 14).
Required Components
The IEP must include:
- Measurable postsecondary goals based on age-appropriate assessments in:
- Education/training (college, vocational programs)
- Employment (competitive, supported, sheltered)
- Independent living skills (when appropriate)
- Transition services — The instruction, related services, community experiences, and objectives needed to reach those goals
- Course of study — How the student's classes align with their postsecondary goals
- Agency linkages — Connections to adult service agencies (e.g., Department of Rehabilitation, Regional Center)
Student Involvement
- The student must be invited to any IEP meeting where transition is discussed
- Students should be encouraged to self-advocate and participate meaningfully
- Student preferences, interests, and strengths drive the transition plan
Age of Majority
- At age 18, educational rights transfer from the parent to the student in California
- The school must notify the student and parent of this transfer at least one year before the student turns 18 (EC §56041.5)
- The student can designate the parent to continue making decisions if they choose
Key Transition Activities
- Career exploration — Job shadowing, career assessments, internships
- Vocational training — CTE courses, work experience programs, community college dual enrollment
- Independent living skills — Financial literacy, transportation, self-care, cooking
- Community participation — Volunteering, recreation, social skills groups
- College preparation — SAT/ACT accommodations, college disability services orientation
Dean/Counselor Role
- Attend transition IEP meetings — Provide input on behavioral and social readiness
- Connect students with resources — Career center, college counselors, vocational programs
- Support self-advocacy — Help students practice speaking up for their needs
- Monitor graduation progress — Ensure students are on track for diploma or certificate
- Coordinate with outside agencies — Department of Rehabilitation, Regional Center, Independent Living Centers
